Activity Description & Educational Objectives


This educational activity is based on the curriculum for iHealth training.  

iHealth is a web-based Disease Management Tool (DMT) that provides a useful method of co-managing patients that are being treated by a Project ECHO partner.

Project ECHO is an innovative health care delivery system which teams up rural health care providers with highly trained medical specialists to co-manage patients with complex chronic diseases.

 Target Audience


This activity has been designed to meet the educational needs of any and all Project ECHO partners. This may also include healthcare providers, including physicians, physician assistants, nurse practitioners, nurses, public health nurses and medical assistants.


Requirements for Successful Completion


In order to receive a Certificate of Completion, participants must complete the activity: view all the iHealth Quick Start Guide,visit the iHealth Webpage, view the iHealth videos, and complete quiz. There are no pre-requisites and there is no fee to participate in this activity.
